Take an active role in your own healthcare by
- Being open and honest about your health history and healthcare needs, including past and current illnesses, allergies, medications and dosages, and family health history.
- Arriving promptly for appointments or notifying us in advance (at least 24 hours) if you need to cancel or reschedule.
- Treating our staff, fellow patients, and visitors with consideration and respect.
- Refraining from smoking on the premises.
- Taking reasonable precautions to maintain your health.
- Following the advice and information provided by your healthcare providers.
- Asking questions to clarify any information you may need.
- Communicating what is important to you in your care.
